10 things to do" to join HYLTON Crew

​

  1. All participants are required to have completed Physical Examination Form (MUST be “Revised April 2016” version) to be completed and signed by qualified medical personnel. Click here for form Please always retain a copy of this form, it should be turned into Mrs. Wernlein-Team Registrar, or your coach.

  2. Novice (new) teammates need to complete a SWIM TEST at Chinn Aquatics (cost @$4), Click here for form Please always retain a copy of this form, it should be turned into Mrs. Wernlein-Team Registrar, or your coach.

  3. All participants need to complete the online insurance waiver fee ($7.75) via USRowing Click here for instructions

  4. All participants need to complete 2 Blue Emergency Contact Info cards. Winter Conditioning Participants have already completed these. These forms can be obtained from your coach or Mrs Wernlein. Please complete and return completed forms to Mrs. Wernlein-Team Registrar or your coach

  5. All participants need to read and sign the MS/HS Rules and Regulations: keep 1 copy, turn the other in to Mrs. Wernlein-Team Registrar, or your coach. Most winter conditioning participants have this on file. This form can be obtained from your coach or Mrs Wernlein. Please complete and return the completed form to Mrs. Wernlein-Team Registrar or your coach.

  6. For Novice (New) teammates: In-Person Concussion Training for all novice participants is REQUIRED. Please note that the student and a parent need to attend this training. Click Here for a copy of the current presentation schedule

  7. Order your Hylton Crew Uniform

  8. For Women’s teammates that are 135 lbs. and under and for Men’s teammates that are 155 lbs. and under need to complete a HIPAA lightweight form (this form will be available to complete on the Ronin website when you pay team dues) and complete Hydration testing at the school. Date and time of testing TBD by school. A link will be available here once dates and times are announced.

  9. All participants are required to do baseline concussion testing at the school. Date and time TBD by school. A link will be available here once dates and times are announced.

  10. Pay 2017 Hylton Crew dues ($425) and complete a few other waivers, you can access the Ronin Website by clicking here February 1st to March 3rd.
